1. Disclaimers

1. By checking this box I understand that the Arnold Nutrition Coaching (“ANC” hereinafter) coaches are certified nutrition coaches and are not medical doctors. ANC and its coaches do not dispense medical advice or prescribe treatment with regard to the services provided by ANC. Rather, they provide education to enhance my knowledge of health as it relates to foods, dietary supplements, and behaviors associated with eating. While nutritional and botanical support can be an important complement to my medical care, I understand nutrition coaching is not a substitute for diagnosis, treatment, or care of disease by a medical or healthcare provider.

2. Participant understands that if he/she is under the care of a medical or healthcare professional or currently uses prescription medications, Participant should discuss any dietary changes or potential dietary supplement use with his/her medical or healthcare provider and should not discontinue any prescription medications without first consulting Participant’s medical or healthcare provider. The Participant should continue regular medical supervision and care by his/her medical or healthcare provider. 

2. Personal Responsibility and Release of Health Care Related Claims

1. The Participant acknowledges that the Participant takes full responsibility for the Participant’s life and

well-being, as well as the lives and well-being of the Participant’s family and children (where applicable), and all decisions made during and after the duration of the Participant’s nutrition and wellness coaching sessions.

2. The Participant expressly assumes the risks of nutrition and wellness coaching sessions, including the risks of trying new foods, and the risks inherent in making lifestyle changes.

3. The Participant releases and agrees to hold harmless ANC, its contractors, employees, and agents from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever arising from the Participant’s participation in ANC’s wellness program. If any action is filed in relation to this release, the unsuccessful party in the action shall pay to the successful party, in addition to all the sums that either party may be called on to pay, a reasonable sum for the successful party’s attorneys’ fees.

3. Payments and Refunds.

1. There are no refunds for payments made to ANC.

The Participant understands that they have agreed to a monthly membership and it is the responsibility of the participant to cancel, payments WILL NOT automatically be canceled.
